(1) Inform the insurance company
When an insurance accident occurs, we should immediately inform the insurance company or salesman by telephone, letter, fax, door-to-door, etc.
(2) Submit application materials
After notifying the insurance company, we should give the insurance company the supporting documents stipulated in the insurance contract, or entrust the salesman or others to do it in writing.
These documents mainly include the following:
① Insurance contract.
② Claim application.
③ The identity certificate of the insured and the identity certificate of the insured.
④ Outpatient medical records and prescriptions.
⑤ Discharge summary and diagnosis.
⑥ Original receipt of medical expenses.
⑦ Detailed list of hospitalization expenses.
⑧ Application form for extension of stay in hospital (the terms indicate that the stay in hospital is more than 15 days).
⑨ Certificate of diagnosis of major diseases.
⑩ Certificate of accident (for example, the insured shall provide valid driver's license and driving license in case of traffic accident, and relevant responsibility identification materials in case of traffic police handling).
Disability identification report (need to contact claims department).
Power of attorney.
Identification of the client.
A copy of the beneficiary's passbook.
Beneficiary's identity certificate, registered residence certificate, and the relationship with the insured.
Application from non designated hospitals.
The death certificate, funeral certificate and proof of registered residence of the insured, issued by the public security department or the medical institution approved by the insurance company, such as death certificate, cremation certificate, cancellation of household registration, etc.
Proof materials related to the nature of the accident: Certificate of accident and industrial accident, hospital death records and relevant medical records, autopsy report issued by judicial and public security organs, etc.
(3) Wait
After submitting the application materials, the insurance company reviews the liability and calculates the amount of compensation. We need to wait for a while.
(4) Collect money
Once the insurance company has completed the audit, it will inform us of the conclusion in writing. We can take the ID card and the written notice to get the insurance money.
